Washington Law Help is a non-profit organization connected to the Northwest Justice Project, who provides access to free forms for a broad variety of legal matters. This organization has a detailed list of documents.

Traveling with Minors
Sometimes children may travel with persons who are not the parent. This can present challenges when it comes to international travel. There are resources for "Travel Letters" for children without parents. These letters are not required to be notarized. It is, however, highly recommended by the US Citizen and Immigration Service (USCIS) to have these documents notarized. More information can be found here. You may also consider a Power of Attorney for Parents.
I am not verifying who the parent of a minor may be. I am merely identifying the signatory and certifying the authenticity of the signature provided on the travel document.
